Inappropriate Nursing Delegation

INSTRUCTIONS:

Goals of this Written Assignment:   Ø  Early in the semester the instructors will be evaluating the writing skills of students and determining which students are on target with college-level writing and who will require additional support for success in writing throughout the remainder of the semester. Coherence of ideas, the ability to use grammar and language to professionally express oneself, and appropriate citation using APA style are all keys to success in writing at a college level. This early assignment is intended, in part, to ensure student success by providing feedback and support early.  Ø  Nursing scope of practice, and the ability to provide clinical judgment by discussing and applying critical thinking in delegation is a necessary skill for a registered nurse. The second goal of this paper is for the student to begin making the transition from the task focus of a CNA to professional nursing practice as the leader of a team of health care professionals. Part of this paper will focus on insights on delegation as a registered nurse according to evidence-based guidelines. Ø  In writing this paper, do NOT use direct quotes from the text or articles. The assignment should reflect your thoughts on the material, not quote the material directly. Ø  DO use the materials in the Nursing Fundamentals course menu “writing and APA helps” to guide your process of in-text citation and referencing of material.    Resources for success: (All are in the Scope of Practice learning plan resources in Blackboard) ·      Assigned reading in Potter/Perry text (required) ·      National Council State Boards of Nursing “National Guidelines for Nursing Delegation” (required) Other resources, if needed: ·      Chapter N6 Standards of Practice  ·      Chapter N7 Rules of Conduct (Navigate to these in the “Wisconsin Administrative Code” folder in the Scope of Practice Learning Plan)   Instructions: Follow the rubric to discuss the following topic(s). APA reference examples for some, but not all, of the resources for success you should be using for this assignment are listed at the end of this document. You do not need to provide a reference or citation for the lecturer/lecture material, or the case study in this assignment sheet.   Read the delegation case study scenario below, and use the NCSBN “National Guidelines for Nursing Delegation” to support your discussion of the following: ·      Discuss 3 ways in which proper delegation guidelines were violated. Include adequate detail for the reader to understand the importance of RN delegation guidelines for safe nursing practice. (at least 1 paragraph per violation)   ·      Highlight your own clinical judgment skills in contrasting how you would have properly handled one of the delegation guideline violations that you identified. Defend your reasoning with the resources for success documents given. This is not a personal opinion, or how you would be “nicer” to the staff, this is clinical judgment supported by the literature.    -------------------------------------------------------------------------------------------------------------------------- Case Study: Inappropriate Delegation The human resources department hires a registered nurse to work part-time evenings and every other weekend on a hospital 42 bed medical/surgical unit. The nurse has been out of school for two years and this is her first job as a staff RN in a hospital setting. Prior to this job she worked in a physician's office. Shortly after completing hospital orientation the charge nurse calls in sick, and the staff nurse has to take over her charge nurse duties. After the nurse's first weekend as charge nurse, human resources receive the following complaints:   From other licensed staff: "It seems like she just picked out rooms to make the assignments. She didn't listen to report first or check the charge nurse report sheet. She didn't state any concerns at that time and seemed more concerned about her own patients and getting her medications out on time. We had never worked together, so I didn’t know her style and she had no idea who I was or how long I had worked on the unit”.  From a Certified Nursing Assistant: "She asked me to do things I have never done before! When she told me to hang the tube feeding, I said I can't do that and she said, “JUST DO AS YOU ARE TOLD." She never came back to see what I had done or how the patient was doing. I had no idea what to do if I had a problem. Scenario adapted from The Official Website of the Executive Office of Health and Human Services (EOHHS) Health and Human Services  http://www.mass.gov/eohhs/gov/departments/dph/programs/hcq/dhpl/nursing/education/faculty-resources/model-curriculum/delegate-supervise/case-study-inappropriate-delegation.html   The following are reference examples.
