1. Identify and describe a situation, problem, or opportunity for improving health at your CbSL site. This can be a situation involving a specific person or client that has made you aware of specific issues or needs among clients, or it can be about an existing or needed program at your site. (4 pts) 2. State the significance of the problem. For example, how it might impact clients, providers or important processes at the site (6 pts.) 3. Identify a theory (nursing or non-nursing) that you could apply to the situation to better understand it and then develop a solution or response to it. Cite the source you used to examine the theory for application here. (6 pts) 4. Provide a rationale for using this theory to address the identified problem. For example, if the problem is about relationship between provider and patient, the theory addresses therapeutic communication or alliance- or if the problem is about assisting clients to modify lifestyle, the theory addresses health promotion. (6 pts) 5. Identify and discuss at least 2 key concepts or components of the theory that you can use to understand the situation well enough to develop a plan of action to resolve the situation or produce an effective response. (8 pts) 6. Describe your idea for a plan of action or resolution, referring to the concepts or theory components you identified above. The plan does not need to be something that you could implement, or something that could realistically be implemented in the current situation. It can be something that is optimum or what you think would be the ideal plan. (10 pts) 7. Aim for between 3-5 DS pages 8. Use APA format (2 pts): a. Cover page b. Running head c. NOTE that any deductions for not completing the required 12 hours of CbSL will be deducted from this grade item.
