INSTRUCTIONS:
The purpose of an evaluation plan is to demonstrate program effectiveness and reveal opportunities for program improvement. This is done by measuring the program’s impacts and outcomes and analyzing efficacy of the program’s processes and procedures. When you developed measurable program goals, you determined the performance thresholds that will indicate program success. The most important part of planning program evaluations is collecting incremental data, throughout the duration of the intervention, so you can capture changes and the development of trends as they occur. After watching the assignment overview, use the assignment template and Microsoft Word to complete the Evaluation and Data Plan (template)Preview the document. Follow the guidance, provided in RED, to complete the assignment (remember to replace RED text in the template with your own entries and change the font color to BLACK!). Based on the implementation plan you developed (see attached), identify the followng for the primary program goal and the related objectives: Specific program elements or indicators that shoud be evaluated or assessed. Source of the data or other information that will be used to assess the specified program element or indicator. Method or intrument that will be used to collect the data. The point in the program where the data will be collected.
