Case Management Plan

Case management is invaluable in helping individuals meet their goals and live better lives. It entails assessing client needs, developing an action plan, and coordinating services (Giardino & De Jesus, 2023). This essay aims to create a case management plan for a 21-year-old individual battling drug addiction by identifying their strengths and areas for improvement, gathering pertinent information, and any specific case-management needs that arise. This case management strives to offer an integrative and in-depth approach that addresses clients' biopsychosocial, spiritual and psychological needs. By identifying strengths and improvement areas I will develop an action plan which promotes independence, self-sufficiency and well being for the patient. It will entail support from healthcare providers, social workers and employment agencies all working collaboratively on one plan.

Strengths and Areas of Improvement

This client possesses several strengths regarding communication skills, the desire to change, and having a supportive family. Their communication abilities allow them to express their needs, goals, and concerns effectively. Additionally, their desire to improve their life is an invaluable asset that is the cornerstone of their case management plan. They have shown a willingness to make positive changes that could significantly increase their quality of life - this motivation is essential in reaching goals and surmounting obstacles along their journey. Finally, the client benefits from having a supportive family unit as another resource. Family can offer emotional support, encouragement, and practical help as needed; additionally, they can assist the client with maintaining motivation while holding them accountable for their actions (Malathum, 2020).

However, there are areas of improvement that need attention. One key area is criminal history which may hinder opportunities for employment or education; substance abuse also has severe adverse impacts that affect many aspects of their life, including physical health, relationships, and legal problems. To address these areas of improvement, the case management plan must include strategies designed to assist clients with their criminal histories and substance abuse problems. Such a strategy could consist of finding resources for job training and education programs, attending substance abuse treatment programs, and connecting with support groups.
